How to Get Started

Step 1: Referral Required

We need a referral and prescription from your child’s PCP or pediatrician to begin services.

Step 2: Intake Contact

Once we receive the referral, our intake coordinator will contact you and send intake forms.

Step 3: Complete Forms

Fill out the intake paperwork so we can better understand your child’s needs.

Step 4: Schedule Appointment

After forms are completed, we will reach out to schedule your child’s evaluation appointment.
